POSITION SUMMARY:

The Family Support Navigator (FSN) is part of the larger REACH (Recovery, Engagement, Access, Coaching & Healing) team. The FSN provides outreach, engagement, case management services, and recovery coaching and community connections to treatment and recovery support resources to family members connected to women in the REACH Program. FSN clients may include male partners, grandparents, LGBTQ+ partners, etc. The primary goal of the FSN is to identify, engage, connect and support individuals impacted directly or indirectly, by substance use disorders. This position will represent ABH® and its services in a positive and professional manner and adhere to ABH®’s best practices, guidelines, policies and procedures as established, promoting ABH® as a leader in behavioral healthcare in all interactions and work.


Essential job duties:
1. Work from a strengths based perspective to provide and coordinate a full array of holistic case management services, including services that help connect clients to services aimed at enhancing their recovery plan while supporting their goals as they develop a safe support network.
2. Assist the clients in developing and promoting healthy connections to children, family, significant others and their community;
3. Assist with accessing quality treatment services in an appropriate setting and with service planning for non-clinical needs including transportation, housing, childcare referrals, accessing community supports and assistance in obtaining basic and urgent needs.
4. Work in the community outreaching and engaging individuals impacted directly or indirectly by substance use disorders or co-occurring disorders, establishing and maintaining community connections, and connections;
5. Provide education, advocacy, coaching and mentorship all with goal of increasing clients recovery capitol and community supports.
6. Provide transportation to clients, using personal vehicles; and have a valid CT license.
7. Maintain internal communications by notifying supervisors with all relevant client or program-based information;
8. Document all recovery management activities within required time frame and in required format including documentation of all client related activities into electronic database as required by DMHAS;
9. Computer-savvy and be able to function on a computer and a smartphone.
10. Attend all required staff, supervision and training meetings;
11. Maintain confidentiality of all client protected health information and adhere to all HIPPA related policies and procedures and demonstrate ethical behavior
12. Perform other tasks/responsibilities as required to support the business operations.
13. Possess an associate’s Degree in Human Services or related field or equivalent combination of 4+ years of lived personal and/or familial experience & a minimum of 2 years of sobriety and willingness to use experience to help others.
14. Recovery-oriented training certification or will complete (within 2 months of hire);
15. Experience working with women in recovery and the issues unique to women to include 12 step models, the impact of gender and sexual orientation on all aspects of treatment and recovery.
16. Compliance with current State of Connecticut and federal health and vaccine requirements;
