Job description:  
POSITION SUMMARY:

To provide services to the Conditional Release Service Unit (CRSU) which is part of DMHAS, Division of Forensic Services. The CRSU serves as a link between the PSRB and Local Mental Health Authorities/agencies providing mental health and forensic services to acquittee on conditional release. The primary goals of the program are to: assist in transportation of the client to their already established clinical services in the community as needed, promote recovery and prevent future criminal justice system involvement to the extent possible while promoting public safety; Interface with community providers during an acquittee’ s Temporary Leave and Conditional Release, consultation to Conditional Release Supervisors on such issues as development and review of treatment plans, risk management issues, PSRB regulations, and DMHAS policies to ensure compliance with treatment and statutory requirements, support linkages with integrated community recovery support services. This position will represent ABH® and its services in a positive and professional manner and adhere to ABH®’s best practices, guidelines, policies and procedures as established, promoting ABH® as a leader in behavioral healthcare in all interactions and work.

DUTIES AND RESPONSIBILITIES:

 Coordination of services with the CRSU and DMHAS Division Forensic Services to adults with primary mental health and/or co-occurring disorder, who have been adjudicated as not guilty because of insanity. Services include but are not limited to the following;
o Trained by DMHAS in the protocols required by Psychiatric Security Review Board (PSRB)
o Coordination with DMHAS Forensic Services staff and the PSRB;
o Linkages with mental health treatment including the Local Mental Health Agencies (LMHA) and affiliates;
o Linkages/coordination with health care providers;
o Service planning for non-clinical needs (e.g., housing)
o Transportation to treatment and other community support appointments or the coordination of transportation
o Transportation to essential appointments when no other arrangement is timely or/and practical
o Linkage to housing agencies and advocacy with landlords when appropriate;
o Coordination with probation/parole officers or other legal issues;
o Linkages with other appropriate support and treatment services;
o Collaborate with family members and other natural supports;
o Provide linkage to interpreter services;
o Linkages to faith communities (e.g. churches);
o Review of client’s forensic packet which will include all relevant legal history and psychiatric and or treatment information and history.
o Coordination of recovery supports as recommended and approved by the PSRB and CRSU
o Attend a PSRB training
o Enter facilities including Whiting Forensic and Dutcher Hall.
 Carry a company cell phone during work hours and respond within 30 minutes of initial call;
 Carry a company issued laptop and jet pack.
 Provide transportation to clients using personal vehicles when no other arrangement is timely or practical;
 Work out of the ABH Middletown office and attend regular and ad hoc meetings with DMHAS Forensic Services staff;
 Adhere to all State and Federal Confidentiality Regulation;
 Comply and work towards goals established by DMHAS;
 Maintain internal communications by notifying supervisors with all relevant client or program-based information;
 Document all case management activities and reports within required time frame and in required format including, but not limited to, documentation of all client related activities;
 Attend all required staff, supervision and training meetings;
 Maintain hard copy of clients’ records (i.e. ROI) with appropriate documentation and signatures;
 Function as a team member of a regional and statewide team, including providing as needed coverage for other members of the team;
 Maintains confidentiality of all client protected health information and adheres to all HIPAA related policies and procedures;
 Demonstrates ethical behavior and cultural sensitivity in all activities involving individuals of diverse backgrounds;
 Performs other tasks/responsibilities as required to support the business operations.

Skills:  
EDUCATION AND EXPERIENCE REQUIREMENTS:

 Bachelor’s degree in Human Services or related field required with a minimum of 4 years work experience in a behavioral health program.
 Demonstrated mastery of principles and practices involved in delivering case management services to behavioral health population with primary mental health or co-occurring disorders required;
 Attends annual Conflict of Interest training.


KNOWLEDGE/SKILLS/ABILITIES:

 Demonstrated work knowledge of substance abuse, mental health services, community resources in the local region; knowledge of cultural diversity, engagement, and ethics;
 Bilingual skills preferred;
 Must have valid driver’s license, reliable transportation and carry minimum insurance coverage of $100,000/$300,000/$100,000;
 Must be willing to transport client in personal vehicles;
 Strong attention to detail; ability to work on multiple tasks and meet deadlines;
 Excellent PC skills with demonstrated experience using Microsoft Office Package (MS Word, Excel, Power Point, Access, Outlook); internet;
 Strong written and verbal communication skills required.
